Community Paramedicine Program

Community Paramedicine is a model of community-based health care services provided by paramedics who apply their education and skills in non-emergency situations, outside of their emergency response and transportation roles.  Our objective is to reduce the number of 911 calls, decrease emergency room visits and hospitalization by providing additional supports for high risk or vulnerable individuals.

The District of Timiskaming Social Services Administration Board Community Paramedicine program is part of the local health system partners and coordinates with existing health services, including residential long-term care, home and community care, primary care, emergency care and other community and social services.  The program is accessible, responsive, proactive and safe. 

HOME VISITS

Home visits are conducted by primary care paramedics who are trained to do fall risk assessments, vital signs, point of care testing, medication compliance, as well as referrals for other services that can benefit the patient and allow a safe home environment. A Community Paramedic will answer questions or concerns from patients when doing home visits while providing education for patients to better manage their chronic conditions. Pertinent patient information can be shared with family physicians allowing for less clinic visits.

REMOTE PATIENT MONITORING

Remote patient monitoring programs are designed to track and trend health conditions for people in the comfort of their own home. This program allows patients to use specialized devices to monitor their vital signs and transmit them remotely to a paramedic. This in turn gives paramedics the insight to predict and prevent exacerbations of medical conditions, while also giving patients the opportunity to better learn self-management of their care and improve their quality of life. 

WELLNESS CLINICS

Community Paramedics assist with various clinics throughout the district, including but not limited to:

  • COVID-19 testing

  • COVID-19 vaccinations

  • Influenza vaccinations

  • Health promotion and wellness

The goals of the community paramedicine program are to:

  • Allow individuals to remain in their homes safely as long as possible by reducing emergency department visits and hospital stays while providing education, prevention and responsive care.

  • Mitigate the illness or disease trajectory by supporting an improved quality of life

  • Provide individuals, families and caregivers piece of mind

  • Minimize waitlist growth and duration with alternative options for individuals who wish to stay home with appropriate care.
